Liability Waiver

Welcome to YINART STUDIO! Please read the following waiver carefully. You must agree to the terms by selecting the checkbox before submitting your registration.

Terms of the Waiver and Release Agreement

By registering for and participating in YINART STUDIO programs, I understand and agree to the following terms:

1. I authorize YINART STUDIO staff to obtain necessary medical services for my child during the program, and I give permission to the physician selected by program staff to render any treatment deemed necessary. I understand that the program does not provide insurance for medical or hospital costs, and I am solely responsible for any such expenses.
   

2. Each person participating in the YINART STUDIO program (for minors, their legal guardian) is deemed to have waived all claims against YINART STUDIO and its employees and agents for any injury, accident, illness, or death that may occur during or as a result of participation in the program.
   

3. I have accurately provided all relevant medical diagnoses, physical/medical limitations, and corresponding medications during registration and have communicated these to the YINART STUDIO staff.
   

4. Please attend all purchased classes on time. In cases of emergency or illness, please notify us at least one day in advance. Each semester allows for a maximum of 2 excused absences; additional absences beyond this limit will count toward the use of class hours.
